Historical CIPA

Video showing you how to enter a historical CIPA. This will help when looking at the Care Plan>CIPA Patient Report OR Stable/Unstable List if you are a new clinic and have documented in a prior system.

1. Introduction

This tutorial guides you through creating a CIPA entry by entering historical dates from the old system, ensuring accurate patient data migration and record keeping.

2. Click "CIPA Form"

Once your patient has been selected. Go to the Care Plan menu and click the "CIPA Form" to begin entering historical dates from the old system, initiating the data entry process.

3. Date

When opening the CIPA form it will always auto populate to todays date.

4. Enter Historical Date

Change this date to date of the last documented Plan of Care completed for the patient.

5. Manually Documented

Mark the checkbox to the left of the Manually Documented field. This will check the Manually Documented boxes for all disciplines for the CIPA.

You'll now see that each discipline is checked!

7. Add Demographic Information

Click "Add Demographics" to input necessary demographic details.

8. Open Demographics Section

When the Demographics form displays, the Patient's condition field will display as Stable by default. If the patient's last documented POC was Unstable, click on the dropdown list and select Unstable. If Unstable is selected, enter the date of when this patient was determined Unstable.

9. Assessment Type

Next select if the assessment was the Patient Initial CIPA (New Patient), 90 Days CIPA (New Patient), Monthly/annual (Stable), Monthly (Unstable), or Modality Change.

10. Sign and Save Form

Click "Sign & Save" to finalize and save the form data.

11. Approve and Lock Form

Click "Approve & Lock" to confirm and secure the form.

12. Select the next patient

You can now choose your next patient from the dropdown menu!

13. CIPA Patients Report

When you go into Care Plan, CIPA Patients Report you'll see the list of patients at that facility and who is ready for a CIPA.
